The M-POWER Ministries Health Center provides FREE primary care services to adult patients without access to health insurance, helping to alleviate chronic illnesses. By receiving treatment to combat chronic diseases, people are better able to fight off illness and ensure their ability to be at work every day.

• Walk-in clinic, no appointment necessary, patients seen on first come, first served basis

New Patient Intake Forms

When you come to the clinic, you will need to bring all of these forms filled out with a copy of your photo ID and a copy of your household proof of income. If you have any questions, please call (205) 595-5959, option 1.
